This program serves those who qualify for Medicaid and Medicare and provides comprehensive care through resident’s physicians, home health and hospice providers.  The program is designed to help our residents remain in the comfort of our homes as long as it is medically feasible and desired. Personalized healthcare gives you or your loved one a happier and better health outcome.

Seniors coping with a medical condition like any of the ones listed below need skilled care, which can also be provided at home:

  • Heart conditions

  • Diabetes

  • Depression

  • Arthritis

  • Osteoporosis

  • Breathing problems

  • Frequent falls

  • Parkinson’s disease

  • Others

Post-Op and Rehabilitative Care / Resident Home Health Care

Our senior health care services are available to our residents as needed. Silver Oaks has teams of external Health Service Directors, Registered Nurses, Licensed Vocational Nurses, and Resident Assistants who are committed to providing the highest level of senior health care services for our residents. They include:

  • Nursing care, including post-hospital or post-surgical support.

  • Medication management to ensure medicines are taken as prescribed and to oversee pharmacy services and medication deliveries.

  • Coordination of physician and rehabilitative therapy services, including the option of in-house visits from physical/occupational therapists and doctors with specialties such as primary care, podiatry, and geriatric psychiatry.

  • Working with families and hospice care providers to assure that residents receive any specialized, comforting end-of-life support that they may need.
